Location: SLCSD Office- 440 E 100 S

In order to register, the child must be 4 years old by Sept. 2, 2008.
Please bring: Original Birth Certificate

Immunization Records

2007 Tax Forms (sliding scale tuition)

Proof of residency (util. bill, etc.)

$15 non-refundable registration fee

Parking Lot Safety

The front of the school is very busy in the morning and afternoon. Please be especially careful at these times. Watch out for children crossing the driveways, do not drive onto the sidewalks, and please do not use a cell phone during these times so that you can concentrate on keeping our children safe. Free Mobile Health Clinic

When: Thursday, April 17,

Time: 9:00 AM – 2:00 PM

Where: Mexican Consulate, 155 S 300 W

This clinic is for individuals and familias who do not have medical or health insurance
